We assess your risk according to the Australian type 2 diabetes risk assessment too.<\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">To assess your risk visit : <br \/><a href=\"http:\/\/To assess your risk visit : http:\/\/www.diabetesaustralia.com.au\/PageFiles\/937\/AUSDRISK%20Web%2014%20July%2010.pdf\" target=\"_blank\" rel=\"noopener\">http:\/\/www.diabetesaustralia.com.au\/PageFiles\/937\/AUSDRISK%20Web%2014%20July%2010.pdf<\/a><\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">If you are at high risk come to us.We evaluate your high risk score.We update your history and undertake physical examination and clinical investigations.<\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">We make overall assessment of your risk factors, the results of relevant examinations and investigations.<\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\"><strong>Your Risk Factors<\/strong><\/p>\r\n\r\n\r\n\r\n<ul class=\"wp-block-list\">\r\n<li>Life style risk factor-smoking, physical inactivity, poor nutrition and alcohol<\/li>\r\n\r\n\r\n\r\n<li>Biochemical risk factors-high cholesterol, high blood pressure, impaired glucose metabolism, excess weight<\/li>\r\n\r\n\r\n\r\n<li>Family history of chronic disease<\/li>\r\n<\/ul>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">We initiate appropriate interventions such as referral to a life style medications program done by Medicare local Bentley Armadale.<\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">We provide you with advice and information.<\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\"><strong>Health assessment for people aged 75 years and older<\/strong><\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">This health assessment includes:<\/p>\r\n\r\n\r\n\r\n<ul class=\"wp-block-list\">\r\n<li>Checking blood pressure, pulse rate, rhythm<\/li>\r\n\r\n\r\n\r\n<li>An assessment of your medications list<\/li>\r\n\r\n\r\n\r\n<li>An assessment of continence status<\/li>\r\n\r\n\r\n\r\n<li>An assessment of your immunization status-influenza, tetanus and pneumococcus<\/li>\r\n\r\n\r\n\r\n<li>An assessment of your physical functions, including activities of daily living and falls risk<\/li>\r\n\r\n\r\n\r\n<li>An assessment of your psychological function including mood and cognition, checking of your memory<\/li>\r\n\r\n\r\n\r\n<li>An assessment of social functions including the availability and adequacy of pain and unpaid help<\/li>\r\n\r\n\r\n\r\n<li>We initiate an appropriate referral for the aged care assessment team for further management.<\/li>\r\n<\/ul>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\"><strong>Health assessment for people with an intellectual disability<\/strong><\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">Assess the physical, psychological, social function of the person with intellectual disability to identify any medical intervention and preventive health care required.<\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">Assessment can be done every year. <br \/>This health assessment includes,<\/p>\r\n\r\n\r\n\r\n<ul class=\"wp-block-list\">\r\n<li>Checking dental health<\/li>\r\n\r\n\r\n\r\n<li>Conducting ear examination &#8211; referral for hearing testing if not had one with last 5 yrs<\/li>\r\n\r\n\r\n\r\n<li>Examination of the eyes-arranging review by eye specialists of optometrist if not done within last 5 yrs<\/li>\r\n\r\n\r\n\r\n<li>Checking height and weight and calculating BMI<\/li>\r\n\r\n\r\n\r\n<li>Assessing nutritional status, growth and development<\/li>\r\n\r\n\r\n\r\n<li>Assess bowel and balder functions &#8211; incontinence, constipation<\/li>\r\n\r\n\r\n\r\n<li>Assess medications &#8211; arrange medication r\/v by the pharmacists<\/li>\r\n\r\n\r\n\r\n<li>Check immunization status<\/li>\r\n\r\n\r\n\r\n<li>Check exercises opportunities<\/li>\r\n\r\n\r\n\r\n<li>Assess the adequacy of support for activities of the daily living for the patient<\/li>\r\n\r\n\r\n\r\n<li>Routine assessment such as breast exam, mammography, pap smears, testicular exams etc<\/li>\r\n\r\n\r\n\r\n<li>Check difficulty in swallowing and arranging gastroesophageal diseases<\/li>\r\n\r\n\r\n\r\n<li>Assess risk factors for osteoporosis, arranging investigations and treatment<\/li>\r\n\r\n\r\n\r\n<li>Assess mental health<\/li>\r\n\r\n\r\n\r\n<li>Assessing there are signs of physical, psychological or sexual abuse<\/li>\r\n<\/ul>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\"><strong>Health assessment for refugees and other humanitarian entrants<\/strong><\/p>\r\n\r\n\r\n\r\n<ul class=\"wp-block-list\">\r\n<li>If you think if you are a refugee \/ other humanitarian entrants you might be eligible for this Medicare benefit<\/li>\r\n\r\n\r\n\r\n<li>Prior to heath assessment we have to confirm the visa status to check your eligibility<\/li>\r\n\r\n\r\n\r\n<li>And also this health assessment can be done with in 1 yr of entry in to Australia<\/li>\r\n<\/ul>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\"><strong>Health Assessment for Aboriginal and Torres Strait Islander People<\/strong><\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">All Aboriginal and Torres Strait Islander People are eligible Under 3 age groups we do the heath assessments,<\/p>\r\n\r\n\r\n\r\n<ul class=\"wp-block-list\">\r\n<li>Child less than 15 yrs<\/li>\r\n\r\n\r\n\r\n<li>Person who is aged between 15 &#8211;
